This paper was dated 1986 by Professor Mallard but dated 1991 on the actual paper.  Short handwritten note to Professor Mallard from Ray Wytch "Prof, A publication in the most prestigious orthopaedic journal." Short handwritten note by Professor Mallard stating 'Please remember that must get more copies!'  Q1-878.
