Arranged chronologically and indexed by name of injured party, each form contains details of the patient, the injury and the circumstances under which it was sustained. In cases of a serious or potentially serious injury, there may be other documents such as reports, witness statements, photographs and memoranda. Telexes stating where there has been a medivac (non-emergency medical evacuation) have been included. Some monthly charts or graphs breaking down the type of injuries sustained or the body part injured.</dc:description>
