Notes were copied from the original case books and continued from 1906. Pre-printed format. Contains patient's name, residence, parish, date of admission, age, marital status, occupation, whether epileptic, suicidal or dangerous, facts of medical certificates, physical condition, personal and family history, progress and treatment.
