This paper was published in the Annals of Neurology, volume 59, number 1, January 2006 and argues that 'in vivo imaging should be a proof-of-the pudding for new therapies'. Professor Kuhl has enscribed at the top of the paper the words "To John Mallard: gifted explorer and good friend, Dave Kuhl"
