| Description | Copies of accident and dangerous occurrences forms reported on MCP-01. Details on each form include the date, time and place, what happened, details of any machinery involved, personal and occupational details of the injured party, the nature of the injury and the Offshore Installation Manager's signature. Some memoranda and/or copies of reports to the Department of Energy where the injury or incident was serious or caused by non-compliance with procedures, also letters concerning the correct reporting of injuries and incidents. Reports are roughly divided into minor incidents involving Total employees, legally-notifiable incidents involving Total employees, and incidents involving Contractors. Forms are then arranged reverse-chronologically within these divisions. Many are carbon copies and some have faded. |
