Gives the name of the patient, the date of previous admissions, the number, the date of admission, sex and class, age, marital status, conditions of life and occupation, previous place of abode, county or parish to which chargeable, dates of medical certificates, bodily condition, form etc of mental disorder, duration, age on first attack, date of discharge, removal or death, and observations. Initial pages have statistics as to the number of beds in each ward,as at June 1929, together with the desired numbers of beds, calculated according to the square footage allowed per patient. |
